Friday, March 10, 1995
Noted 8 pound weight gain at Rehab. B/P checked after warmup and told to sit - until sent home. Dr. Stark notified. His nurse said to come in Monday.


Saturday, March 11, 1995
Felt bad all day: Short of breath & minor chest pain.


Sunday, March 12, 1995 - ZAP/3
No improvement. When AICD fired at 8:30PM, called EMS. Taken to Morton Plant E.R. 5-8 PVC/minute. Admitted for observation.


Monday - Thrursday, March 13 - 16, 1995
Various tests performed: AICD interrogation - Therapy appropriatey delivered after V-tach detection. Dr. Hazlitt: "Pleased with operation of AICD."
Mugga Scan - Now showing 34% ejection factor.
Chemistry - No substantial deviation from norm.
Dr. Murbach checked suture sites. His PA removed a lower incision stitch.
Holter Monitor - No new significant cardiac electrical irregularities.
Dr. Stark: "This was Conjunctive Heart Failure. Expect it from time to time rest of my life. Lucky to have gone this long without seeing it."

10AM: Got dizzy, followed by AICD discharge. OK after single "therapy" dose. Hazlitt's on call physician: No major concern for now.


Tuesday, June 13, 1995
AICD Interogation: 2PM - Morton Plant Pacemaker Clinic: Therapy on June 11 was appropriate. Two other incidents broke (resolved) prior to delivery of therapy.


Monday, June 26, 1995
Sternum chest pain nearly uninterrupted for more than a day, but heart sounds fine.


Tuesday, June 27, 1995
Saw Dr. Stark. EKG: Nothing significant. Boost Zantac+1 for a few days.


Sunday, August 27, 1995 - ZAP/5
3:10PM: Got dizzy after strenuous work on hot porch, followed by AICD discharge. OK after single "therapy" dose. Hazlitt's on call physician: No major concern for now.

2PM: AICD Interogation at Morton Plant Pacemaker Clinic: Therapy on June 27 was appropriate. One other incident on June 30th broke (resolved) prior to delivery of therapy.

Sunday, November 19, 1995- ZAP/6?
I had a nasty bout with influenza, which included elevated temperature to 102.7, extremely frequent and high volume diarrhea, and significant vomitting. During a nap, I awoke to what I thought was the conclusion of an AICD "therapy session." Called Dr. Stark and he suggested to guard against dehydration and electrolytes embalance.


Wednesday, November 22, 1995
10PM: Significant pain in sternum area and down left arm and hand. Took a nitro and pain subsided for less than ten minutes. Repeated process twice and called Dr. Stark. Dr. Abbott on call and told me to go to Helen Ellis immediately and get an EKG.
Nothing significant on EKG or Enzyme tests. However, PT level was lower than acceptable. Dr. Abbott told me to double Monoket doseage for a few weeks, then get another PT test.

No "Therapy" since last interrogation. No abnormal patterns (V-Tach V-Fib, etc.) in past three months.

Appointment with Dr. Carson - annual physical. Things look good, but would like to see me reduce some "non-critical" meds. Reduce Paxil 1/week to eliminate in 7-weeks. Reduce Xanax to 2/day for two weeks, then one/day. Reduce Zantac to only "as needed." All other stay the same.


Thursday, January 26, 1996
Began having "coughing jags" a few hours after going to sleep. Really got nasty by Tuesday, January 31st, called Dr. Stark: Sounds like Congestive Heart Failure again. Come in tomorrow.
